Why clinic owners need a way to stay in control without staying constantly available
By Dr. Unni Menon, Founder, Ekko Medical
It's 10:47pm.
You're not at the clinic. You haven't been for hours. But you're still there — in the way that every doctor knows, the way that never fully goes away.
Your phone lights up.
A patient.
Maybe it's a question about medication. Maybe it's a concern after a procedure. Maybe it's an appointment request that could easily wait until morning.
And that, I think, captures one of the least discussed problems with patient communication today.
Doctors want to switch off.
Clinic owners can't quite let go.
Very often, they're the same person.
The system we built to protect ourselves
WhatsApp solved a genuine problem for healthcare.
Patients already use it. It's familiar. It's immediate. And particularly across Southeast Asia, telling patients “just WhatsApp the clinic” is far easier than asking them to learn another communication system.
So clinics adapted.
A WhatsApp Business account for reception. Perhaps another phone for another location. Staff members handling messages. Maybe several devices and accounts operating under the clinic's name.
It creates distance between the doctor and the constant stream of messages.
But distance isn't the same as control.
The clinic owner may no longer answer every message personally, but another concern takes its place:
Did someone reply?
Who replied?
What did they say?
Was a clinical question escalated appropriately?
Is there something happening in my clinic that I should know about?
You can put the phone down.
But you can't necessarily stop wondering.
The tension isn't really about WhatsApp
For a while, when we were building Ekko, we framed the problem as:
Healthcare needs an alternative to WhatsApp.
The more doctors we spoke to, the more we realised that framing was wrong.
One message kept coming back:
“My patients are on WhatsApp. They're not going anywhere.”
And they're right.
Why should a patient who already knows how to contact their clinic have to download another app simply because the clinic needs better governance behind the scenes?
An elderly patient shouldn't have to change their behaviour.
A worried parent shouldn't have to remember which healthcare app to open.
A post-operative patient shouldn't have to create another account just to ask a question.
So the problem isn't necessarily getting patients off WhatsApp.
The problem is what happens on the clinic side of WhatsApp.
What a clinic owner actually needs
When I strip away the technology, I think clinic owners are asking for four fairly simple things.
Visibility. One place where the appropriate team members can see patient conversations rather than communication being scattered across accounts and devices.
Accountability. A clear record of who responded, what was communicated and when.
Boundaries. A system that allows the clinic to establish how after-hours communication should be handled rather than requiring the doctor to personally defend that boundary every evening.
Oversight without constant involvement. The ability for a doctor to check what's happening when they choose to — without needing to participate in every conversation.
That's a very different proposition from simply replacing one messaging app with another.
It's about allowing the doctor to be present without always being available.
Patients stay on WhatsApp. The clinic gets Ekko.
That insight changed how we're building Ekko.
The goal is no longer to force patients away from a communication channel they already understand.
Patients can continue messaging the clinic through WhatsApp.
Behind that familiar patient experience, Ekko provides the structured clinic-side layer.
Instead of patient communication living across different phones and accounts, the clinic team can work within a system designed around healthcare communication, with clearer roles, visibility and accountability.
And we're taking the same approach to the clinical systems clinics already use.
Ekko is now integrated with Plato EMR, allowing us to connect patient communication with the existing clinic workflow rather than creating yet another isolated system.
That matters.
Because a communication platform that doesn't understand the patient record risks becoming another silo.
The longer-term opportunity is much more useful:
Patient on WhatsApp → clinical team in Ekko → patient context in the EMR.
The patient doesn't need to understand any of that.
They just message their clinic.
The real off-switch
Technology can't stop a doctor from caring about their patients.
Nor should it.
But it can change the way that responsibility follows them home.
Imagine looking at your phone at 10:47pm and knowing:
The clinic received the message.
The appropriate staff member can see it.
You'll be able to see what happened.
And if something genuinely requires your attention, there is a structured way for it to reach you.
Otherwise, tomorrow is okay.
That's the off-switch I think many doctors are actually looking for.
Not disconnection.
Confidence.
Confidence that things are being handled even when they're not personally handling them.
Better communication shouldn't mean more communication
Burnout in medicine isn't always one catastrophic event.
Often it's accumulation.
One late-night notification.
One quick reply from the sofa.
One message during dinner.
One “I'll just check this before bed.”
Repeated hundreds of times.
The answer isn't to make doctors care less.
And it isn't necessarily to tell patients to stop using the communication tools they already prefer.
It's to build a better system around those conversations.
Patients stay where they're comfortable.
Staff get structure.
Clinic owners get visibility.
And doctors finally get something that's surprisingly difficult to achieve in a connected world:
the ability to step away without losing control.
Your patients can stay on WhatsApp. Your clinic can work differently.
Ekko Medical is building the structured communication layer between patients, clinic teams and the systems clinics already use.
